- If previously awarded a scholarship and/or financial aid in Health and/or Kinesiology, when and what award did you receive?
- List any additional scholarships and/or financial aid you have received while attending the University of Utah. Please clearly indicate if any are loans that require repayment.
- List up to 10 honors, formal recognition and /or awards and the year they were received in chronological order.
- List and describe up to 5 relevant work experiences in chronological order.
- List and describe off-campus service or volunteer activities related to health and kinesiology in which you have participated. Examples may include the U-FIT Program, Special Olympics, youth sports programs, volunteer health screenings, health-related community service projects, research involvement, etc. Be sure to highlight any leadership roles for these activities.
- List and describe any campus and/or departmental service or volunteer activities related to health and kinesiology in which you have helped plan, organize, implement or evaluate. Be sure to highlight the leadership roles for these activities.
- List up to 10 professional activities and/or affiliations. These may include memberships in professional organizations (such as SOPHE, ACSM, ACE, NSCA, UPHA), conferences attended, professional/research presentations, etc.
- Please write a personal statement so that the Scholarship Committee can have a better sense of who you are and why you are deserving of a scholarship. As part of your statement be sure to include what interested you in the particular emphasis you have chosen, your educational goals, and your career goals. Please be sure to address how you meet the specific criteria for the scholarships you are applying for in this statement; this may include academic excellence, financial need, commitment to your future profession, commitment to an active and healthy lifestyle, desire to be of service to society, and/or commitment to promoting inclusivity and diversity. You will only submit one personal statement, even if you are applying for multiple awards.
- One Letter of Recommendation is required for your application. You will be asked to provide the name and email address of your recommender in the Recommendation Form; an email will be sent to them with directions for submitting the letter. The letter must be from any higher education faculty member who can address and/or assess your academic abilities OR from any higher education faculty member or school/work/volunteer supervisor who can speak to your aptitude and character.
