Occupational Therapy Scholarship Application

This is the general application for all of the scholarships offered by Occupational Therapy. Applicants will be matched to a scholarship based on the criteria for each of the following:

  4. List any scholarships you have been awarded previously by the Department of Occupational and Recreational Therapies
  5. Describe your community/volunteer experience including approximate number of service hours. Do NOT include shadowing hours or field work experiences.
  6. Please list any leadership in professional or student OT organizations (e.g. SOTA officer, participation in UOTA or AOTA) and describe how you fulfilled your responsibilities to the organization. If this does not apply to you, please indicate with N/A.
  7. Please write about your commitment to your education to become an occupational therapist and/or your commitment to the Occupational Therapy profession. This can include service to persons in the community who are experiencing occupational barriers OR demonstration of advocacy skills for persons who are experiencing occupational barriers.
  8. Describe your commitment to or interest in working with individuals with neurologic conditions, particularly any involvement with the Skaggs Medical Gym
